TERMS AND CONDITIONS: By submitting payment information through this site you agree to the terms and conditions of this Agreement and any documents incorporated by reference. You further agree that this User Agreement forms a legally binding contract between you and Express Recovery and that this Agreement constitutes 'a writing signed by You' under any applicable law or regulation. Any rights not expressly granted herein are reserved by Express Recovery. By clicking on the "Sign In" button you are accepting these terms and any applicable fees for using this service. Please remember to print this form and the receipt for your records.
Refund Policy: Express Recovery, Inc. has no obligation to refund consumers for any reason once a payment has been authorized. However, in the event of an overpayment due to adjustments or previous non-reported payments, Express Recovery, Inc. will attempt to refund the amount of the over payment back to the original Credit/Debit/HSA card that was used to facilitate payment. In the event that the original card is no longer valid, Express Recovery will send a check via regular United States Postal Service first class mail to the original authorized payor. Refund checks are mailed monthly by the 15th of the month.
PRIVACY POLICY: Personal Information: We do not sell, trade, rent or otherwise share for marketing purposes your Personal Information with third parties. In general, the Personal Information you provide to us is used to help us communicate with you or provide reasonable safeguards to verify identity. Non-Personal Information: In general, we use Non-Personal Information to help us improve the Service and customize the user experience. We also aggregate Non-Personal Information in order to track trends and analyze use patterns on the Site.
Notice: You agree that we may provide notices to you by using one or more of the following methods: Posting them on this Site; Emailing them to an email address that you have provided; Sending them via text message to a mobile number you have provided; Or by mailing them to a postal address that you have provided. You further agree that we may contact you via text, email or phone and that we may utilize automated dialers, prerecorded messages, or AI. All notices by any of these methods shall be deemed received by you no later than twenty-four (24) hours after they are sent or posted, except for notice by U.S. mail, which shall be deemed received by you no later than five (5) calendar days after it is mailed. You may provide notice to us at PO Box 26415, Salt Lake City, UT 84126. Notices sent to us will be deemed received within 1 business day of receipt of such notices.
